For Families & Individuals

What does A Care Assistance Living do?

A Care Assistance Living helps individuals and families find and explore suitable housing options. We help connect people with housing providers based on their needs, preferences, and preferred location.

How does A Care Assistance Living help me find housing?

We begin by learning about the individual's housing needs, preferred location, and living arrangements. We then help identify and recommend potential housing options and connect families with participating housing providers to discuss the details.

Does A Care Assistance Living own or manage the homes?

No. A Care Assistance Living does not own or manage the homes. We help individuals and families find suitable housing by connecting them with independent home providers who may be able to meet their needs.

What information do I need to get started?

To help us understand your housing needs, it is useful to prepare the following information:

  • Basic information: Your name and contact details.
  • Who needs housing: Yourself, a parent, a family member, or someone you assist.
  • Preferred location: The city or area where you are looking for housing.
  • Type of housing needed: For example, an adult care home, assisted living, independent living, or a shared living arrangement.
  • Care and support needs: Any assistance needed with daily activities, mobility, supervision, or other support.
  • Preferred move-in date: When you or your loved one hopes to move.
  • Budget: Your estimated monthly housing budget, if known.

You do not need to have every detail ready before contacting us. We can guide you through the information step by step.

What types of housing and care can you help us explore?

We can help you explore options such as assisted living, adult care homes, family care homes, memory care, respite care, high-acuity care, and independent or shared living arrangements. Services, requirements, costs, and availability vary by provider and location. Learn more about care options.

Is there a cost to families?

Contact us to learn more about how we work. We're happy to explain our process and answer your questions before you decide on any next steps. Any costs for housing and care are set by each housing provider and discussed directly with them.

Can you guarantee a placement or a specific home?

No. We help you explore options and connect with participating providers, but each housing provider makes its own decisions about admission, services, and availability. You always decide whether a home is the right fit for you or your loved one.

I'm not sure my loved one needs care yet. Where should I start?

How do connections with families work?

A Care Assistance Living learns about the housing needs of individuals and families and looks for potential matches among participating providers. When a possible match is identified, we help connect the family and provider so they can discuss the home's accommodations, services, requirements, costs, and availability.

Participation does not guarantee referrals or placements.

How do I apply?

Complete our short provider application. You'll tell us about your home, the types of care and support you provide, and the payment sources you accept. Our team reviews each application and follows up with next steps.

How do I keep my availability up to date?

Approved providers receive a partner portal login where they can keep their availability, care levels, and contact details current. Log in to the partner portal.

Is there a cost to participate?

Contact us to learn more about how participation works. We're happy to answer your questions before you apply.

For Referral Partners

Who can become a referral partner?

A Care Assistance Living welcomes social workers, case managers, community agencies, nonprofit organizations, healthcare and community support professionals, and other individuals or organizations that assist people with housing-related needs.

If you help individuals and families explore housing possibilities, we welcome you to connect with us and learn about partnership opportunities.

How does the referral partnership work?

Referral partners can introduce individuals or families who may need help exploring housing options to A Care Assistance Living. With the individual's permission, the referral partner can connect them with our team.

A Care Assistance Living will learn about their needs, explore potential housing matches, and help connect them with participating housing providers.

Any formal referral arrangements will be discussed and agreed upon separately.

How do I submit a referral?

Approved referral partners can submit referrals online through the partner portal and track their status. To get started, join as a referral partner. You can also call us at (419) 377-2399.

Do I need the individual's permission before making a referral?

Yes. Please only share an individual's information with their permission, or the permission of their authorized representative.

What information should I include in a referral?

General information is enough to get started: contact details, the preferred location, the type of housing being considered, general support needs, the timeline, and any known budget or payment sources. Please don't send medical records, diagnoses, or ID numbers such as Social Security or Medicaid numbers through our website. We'll let you know if anything else is needed.
